As reported by many people who decided to come out of Vault 111 again (thanks to TrueAchievements), Fallout 4 isn’t quite as successful on Xbox as it should be. This seems to have been an issue since the last days of March, and since then, the poor Sole Survivor on Microsoft’s console hasn’t been able to save Preston Garvey or play in their P.I.P. Thankfully, Bethesda appears to be aware of these issues and has released a support article stating that the developer is “currently investigating” the issue. If you encounter an achievement that should be unlocked but isn’t, this article recommends that you do a few checks before reporting it as a bug to make sure the problem isn’t your fault.

You know, making sure you’re actually doing the thing that’s supposed to unlock the achievement, or checking that you’re not having any network connection issues when you’re supposed to be unlocking it. The third suggestion, which is to make sure your trophy/achievement lists are synced correctly, might be a little more useful, and the support post details the steps required to do this.
“If you’re confident that you’ve done everything necessary and refreshing doesn’t fix the issue, you will need to try the trophy/achievement again,” Bethesda concluded. So hopefully this issue will be resolved soon, especially since PC and PlayStation players appear to be unaffected.
